bibliography update

I have, as promised, now updated the web version of the bibliography of
secondary literature. The usual "as is" proviso applies. I am working on
making it accessible via a database-www interface, but learning database
theory and sql and cgi and perl at the same time as writing up a thesis is
not exactly a speedy process.

Main new items - not much, a few books from 1997,1998. Some articles and
chapters I dug up on First Search today, some US/Canadian dissertations
from Dissertation Abstracts.

Hope someone finds it useful other than me having typed all those 1261
references in ;-)
